Steps to Take

Step 1. Visit the following website.

Step 2. Under the section titled “File a complaint,” you can call the number (888) 700-9995 or click on the “Submit an online complaint” link.

Step 3. If filling the online complaint form, you will be taken to a new page with the form.

• Here, you will be asked to describe the problem, name the best time and date for investigation, and state the location of the complaint. 

• You will also have the opportunity to provide any supporting documentation that you may have, although this is not required.

• There is also a section where you can provide your information, but you can choose to write “Anonymous” instead of providing your name. 

Step 4. When you are done filling in the information, click the “Submit” button.
